About this Event

Healthcare was built around visits yet chronic disease happens every day. For millions of people living with heart failure, hypertension, diabetes, and other chronic conditions, what happens during the weeks or months between doctor appointments can determine whether they remain stable or end up in the emergency room. Brook is building what it calls the “operating system for continuous care,” giving health systems the technology, clinical teams, and infrastructure to monitor and support patients beyond the traditional office visit.

Brook is building a continuous-care platform designed to keep health systems connected to those patients every day. Brook combines connected monitoring, AI-powered longitudinal intelligence, and human clinical care teams in a fully managed service integrated into health system workflows and electronic medical records. Its CAPSULE technology maintains ongoing patient context, helping prioritize who needs attention while automating routine work.

The clinical results may be the most compelling part of the story. In a UMass Memorial Health propensity-matched study of chronic heart failure patients, participation in a Brook protocol-based remote monitoring program was associated with 27% lower odds of inpatient admission, 26% lower odds of emergency department visits, 21% lower odds of 30-day readmission, and 51% lower odds of one-year mortality.

Join our executive briefing with Brook CEO, Oren Nissim, to learn more about the company that is generating $11.5 million in ARR, serving 15,000+ patients with more than 20 enterprise partners, and is poised for significant growth..
